Foundation Health Quality and Population Health Director in Fairbanks, Alaska

The Quality and Population Health Director leads and directs the improvement of health outcomes, quality of care, patient safety, and value of care for the population of patients served by Foundation Health Partners. Develops system-wide programs and policies focusing on quality, safety, and population health. Develops and implements clinical guidelines for care designed to improve outcomes while managing costs. Tracks and presents results of improvement efforts and ongoing measures of clinical processes to management. Leads significant clinical event investigations, root cause analyses (RCAs), and peer reviews. Develops programs aimed at the prevention of disease and promotion of health. Recommends clinical initiatives, policy implications and best practices after reviewing health literature and statistics. Manages specific population health programs from planning and design through implementation and evaluation.

  1. Analyzes incidents, sentinel events, and risk outcomes with administrators, physicians, clinical leaders, and staff to identify improvement opportunities utilizing quantitative techniques and knowledge of health care operations and systems thinking. Obtains and uses literature, best practice, and benchmark data whenever possible. Manages programs for the FHP System, to include hospital, skilled nursing, and clinics.

  2. Manages significant clinical event and root cause analysis, including peer review, process improvement, tracking and reporting of processes, policies and procedures. Uses improvement methodologies (i.e. PDSA, Lean) to lead teams in quality improvement. Eliminates sources of error: identify causes of variation, develop and implement systems and processes to facilitate effective clinical practice and error reduction.

  3. Manages day-to-day activities to improve population health outcomes, quality of care and value of care. Designs and implements data driven programs that assist in the identification and health management of high-risk populations. Prepares the organization in readiness for value-based care.

  4. Establishes and oversees the development and implementation of short- and long-range goals and objectives and determines the optimal progression to obtain these goals. Reviews, analyses, and reports of various activities to determine progress toward stated goals and objectives. This position supports the development and achievement of department budgets in conjunction with these goals and in accordance with company standards.

  5. Serves as a content expert for designing and providing formal and informal training for physicians, clinical leaders, staff, and other constituencies related to healthcare quality and patient safety. Provides advice and counsel for leadership at all levels of the organization.

Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards. MINIMUM QUALIFICATIONS

Must possess a strong knowledge and understanding of clinical care and operations as normally obtained through the completion of a bachelor’s degree in nursing, business, health care or related field, and either an RN license in state worked OR a clinical background with current license/certification or equivalent experience.

Professional in Healthcare Quality Certification (CPHQ) must be obtained within the first year of this role.

Must possess a strong knowledge of clinical operations and leadership as normally demonstrated through 5 years of progressive management experience relevant to patient population, quality management, and patient safety. Must demonstrate excellent human relations skills and have the ability to develop working relationships with physicians, patients, and other healthcare team members. Knowledge of the requirements of applicable regulatory and accreditation agencies. Must also possess demonstrated skill in problem analysis, project management, conflict resolution and oral/written presentations.

Must possess demonstrated flexibility in responding to the needs of multiple constituencies with a service oriented philosophy. Demonstrated ability to lead and facilitate interdisciplinary teams. Must also possess demonstrated skill in problem analysis, project management, conflict resolution and oral/written presentation. Requires ability to balance and manage multiple projects.

PREFERRED QUALIFICATIONS

Experience with Just Culture preferred.

Master’s degree preferred.

Experience with TeamSTEPPS strategies preferred.

Additional related education and/or experience preferred.
